Class 1 Obesity: Don't Worry, Be Happy? | Dr. Sharma's Obesity Notes

According to a landmark paper in this week?s issue of JAMA, I now realize that I need to gain about 75 lbs to get the most out of my pension plan.

Indeed, that is just about how many extra pounds I would need to pack on to achieve a BMI of 33, which would finally put me squarely in the Class 1 obesity range. That, according to this extensive review of the literature, would bestow me with the longest life expectancy.

Unfortunately, this will not be easy - If I recall correctly, Morgan Spurlock (the fellow in Super Size Me) only managed to gain a measly 25 lbs - and I recall how hard he had to work at this.

Or, could it perhaps be that the results of this paper are so obviously nonsensical, because the researchers asked the wrong question?

After all, who still cares about BMI?

I can only assume that my US colleagues were far too busy running their analyses to have time to read the Canadian Medical Association Journal (CMAJ or Canada?s version of JAMA), which happened to publish our analysis of data from the US(!) National Health and Nutritional Education Survey (NHANES) on this issue.

As readers of these pages may recall, our analysis of applying the Edmonton Obesity Staging System (EOSS) to two independent NHANES datasets, essentially showed that when it comes to mortality, what matters is how ?sick? your are and not how ?big? you are.

If you have a weight-related health problem (i.e. EOSS 1+), you die, if not, you don?t - end of story!

Neither BMI nor waist circumference were much use in predicting mortality - but whether or not you had hypertension, diabetes, or sleep apnea was.

As we outline in our paper, not only would BMI overestimate health problems in millions of US citizens, it would also completely miss about 25 million Americans, who do have weight-related health issues, despite falling well below the BMI 30 obesity range.

Perhaps, after this paper, we can finally lay BMI to rest and stop trying to predict people?s health with just scales and measuring tapes.

Hopefully, the only landmark that this paper leaves behind is a tombstone - BMI - RIP!

Let us now get back to actually taking a good medical history, doing a thorough physical exam, and running some tests before declaring someone too light or too heavy for their health.

And in the meantime, let?s not forget that prevention best starts by not losing sleep over your weight unless you have to.

India's Infosys denies report of firing 5,000 staff

(Reuters) - India's Infosys Ltd said on Friday a newspaper report it was planning to fire up to 5,000 poorly performing workers was "wrong", although it encourages "chronic underperformers" to leave as part of its routine staff management.

The Economic Times newspaper had earlier said Bangalore-based Infosys, India's second-largest software services exporter and an icon in the country's $100 billion outsourcing sector, was sacking up to 5,000 workers to trim costs.

Infosys, which has more than 150,000 staff, said there was no mass lay-off planned at the company and the number of underperformers that could potentially leave was "significantly lower" than the 5,000 mentioned in the paper.

"We have a robust performance management system that includes structured appraisals and performance feedback," it said in a statement. "This is done regularly and is not a one-time event."

Infosys, for years an investor favorite for exceeding its earnings targets, has struggled recently as its big customers cut costs, missing its own revenue guidance in three of the past four quarters.

The software exporter may cut its revenue forecast for the year to March when it reports its December quarter earnings on January 11, as U.S. business clients put off spending and balk at signing big deals.

With about 60 percent of its business in the United States, Infosys is particularly vulnerable to swings in U.S. corporate sentiment and has been hit hard by spending deferrals by the companies in the world's largest economy.

However, Infosys executive co-chairman S. Gopalakrishnan was quoted by other media reports on Friday as saying 2013 would be better than last year for India's export-driven information technology industry.

Gopalakrishnan was quoted as saying brighter prospects for the United States and China would help the IT sector, as he addressed an event for the Infosys Science Foundation on Thursday.

Infosys shares were up 0.1 percent at 0829 GMT at 2,339 rupees, while the broader market index was down 0.2 percent and the IT sub-index was trading 0.5 percent higher.

Corning announces Gorilla Glass 3

Once, twice, three times a gorilla? Corning's primate-themed glass is ready for a third go-round, and as you'd expect, it's even tougher than the first two iterations. Gorilla Glass 3 has been improved at the molecular level, incorporating a proprietary feature called Native Damage Resistance (NDR). According to Corning, NDR reduces the propagation of flaws, the appearance of scratches and does a better job of maintaining the overall retained strength of the glass. As a result, GG3 claims a three-fold improvement in scratch resistance, 40 percent reduction in the number of visible scratches and 50 percent boost in retained strength after the glass becomes flawed. None of this actually means your phone or tablet will survive a five-foot drop onto concrete, granted, but you'll at least feel more comfortable keeping your keys somewhere close by. Parents of journalist missing in Syria make public plea for his release

The parents of James Foley, age 39, who was kidnapped Syria while on assignment as a videographer for Agence France-Press have had no news of their son in six weeks. In 2012, 28 journalists were killed in Syria.?

From their New Hampshire home, the parents of a foreign journalist who has been missing in Syria since he was kidnapped more than a month ago appealed to his captors for compassion and any information about their son's health and welfare.

Thirty-nine-year-old James Foley was kidnapped in northwest Syria by unknown gunmen on Nov. 22, his parents said Thursday. He was in the country contributing videos to Agence France-Press, which has vowed to help secure his release.

Foley's parents, John and Diane Foley, decided to hold a news conference at their home to make a public plea to his captors because the Foleys haven't received any information about their son in six weeks.

"We just don't know anything," Diane Foley said. "We don't know who has him."

They have kept the Christmas holiday lights on in the windows of their Rochester home as a vigil for their son's safe return.

"I appeal to the people who have Jim to let us know where he is and to help us secure his release," John Foley said. "We just pray that he's released."

Twenty-eight journalists were killed in Syria in 2012, prompting the Committee to Protect Journalists to name Syria the most dangerous country in the world to work in last year.

The Foleys would not discuss any detail about efforts through the U.S. government or otherwise to secure their son's release. Not long ago, they lived through the same anguish and fear over their son's safety.

In 2011, Foley was held by government forces in Libya while covering that country's civil war. Another journalist ? South African photographer Anton Hammerl ? was shot during their capture and left to die in the desert. Foley and another journalist were released after six weeks.

"I'll regret that day for the rest of my life," James Foley told The Associated Press in 2011. "I'll regret what happened to Anton."

Foley has worked in a number of conflict zones in the Middle East, including Syria, Libya and Iraq. He and another journalist were working in the northern province of Idlib in Syria when they were kidnapped in November near the village of Taftanaz. Agence France-Press Chairman Emmanuel Hoog has said the agency is doing all it can to secure Foley's release.

"He's passionate about giving life to stories of people in conflict areas," Foley's father said Thursday.

Asked if Foley had any reservations about going to Syria, Diane Foley replied softly: "Not enough."
